As a member of the IDT, the Nurse Practitioner works under the general direction of the Medical Director, the attending physician and collaborates with the interdisciplinary group in providing skilled nursing assessments, planning, and care in order to maximize the comfort and health of both patients and their loved ones/caregivers.
Provide high quality care to primary and palliative patients in assigned territory with incredibly LIW caseload of 14 - 15 patients per weekAssess patient's health by performing physical examinations; obtaining, updating, and studying medical histories and creating the best plan for care.
Effectively communicate with office staff and clinical operations across various internal platformsProvide high quality, patient centered care to a panel of patients Attend quarterly medical staff meetingsDevelop impressions, diagnosis, and treatment plansFoster trust and consult with patients, families, and caregivers during visitsOrder diagnostic testing as medically necessaryPrescribe medications supplies and appropriate medically necessary interventionsInterested in hearing more?
